If you don't want to do coding, you are in the wrong newsgroup. This one is
about programming.
There are a number of ways to create and use clickable checkboxes in
documents / templates. Some require protected or locked forms, others do
not. Those in protected forms can only have an X to check the box. Othere
can have anything you can find the symbol for. Take a look at the Checkbox
template available at

of these.
What you are talking about is what Word calls an "online form." Check this
in help. For more about online forms, follow the links at

When programming forms to allow the user to add formfields, I have found it
useful to save an AutoText entry of the field and use vba to insert that
AutoText entry rather than hard-coding the field itself. For me, this makes
it easier to make changes.
