running parameter qry from form...

I'm finding that running this query is sometimes deleting the first several
fields of a single record in my table. What would be causing this? It is a
Select Query. Here's the SQL for it (same mdb as my other posts, but the SQL
I posted in the other most recent post had a lot of "False"s in it (I copied
it after running it, so maybe that's why), but here's the actual SQL:

SELECT [Product Keys].[Content/Project File Name], [Product Keys].[File
