Runtime Error '9'

I have added a custom template to Visio 2003. Each time I start Visio it
gives an Runtime Error (Error '9') & says "Subscript out of range".

I have added User forms to take Properties of the shapes on the drawing.

Those worked properly on Visio 2002, But now These forms don't open giving
an error massege saying "Could not load an object decouse it is not valid on
this machine"

If u have any Idea Please Help me
