Safety Alert Document

Hello All,

I work in a plant with 5 major production lines (automotive). I am looking
for some form of template that would go to the safety commity. It should
have a description of the safety concern the area, and so on. Also a spot to
insert a digital photo (not sure how to do that).
Anything to help me get started would be appreciated! I would not expect a
complete template just a kick start :)

I have a safety document on my web page. I think it is pretty good. Scroll down
below the funny money and church envelope. It is a Word document. I have
highlighted the link in yellow.
I made a booklet years ago with this document with images for an Insurance
Safety Engineer.
