Save data to Database, email results, return confirmation page

I have a form that collects data. I would like that data to be sent to a db
so that a "receipt" number can be generated and returned on a confirmation
page. I would also like a notification email to be sent to the form
moderator to prompt an action on their part.

I have looked through this formum and have found, and tried, multiple ways
to accomplish two out of the three, but I really need all three to work
together. I have also viewed, and tried, most of the "links" offered here as
suggestions. Is what I am seeking even possible?
