Save word doc into SQL

Not sure if this is the right forum-

I want to create a document that will post onto our
intranet with Sharepoint. Will want the user to fill out
new hire form that will save into SQL. Not sure how, if
it can be done......Unsure pls advise.

Best to consult with a web developer. They'll likely advise that you
avoid using Word as interface, and create a web-based form that
communicates directly with the SQL server db.
