screen locked after clicking contact in Outlook form won't open?

When clicking on a Contact, the message is custom form could not be opened.
Unable to get a response no matter where I click on the screen. I have run
the repair on both Microsoft Office XP Professional in Control Panel and the
repair in Outlook without success on this problem. Message is also, see
Administrator--I am the only person on this personal computer. Can you help?
