Select Style in TOC Options, but not in TOC

I went into Insert> Reference> Indexes and Tables> TOC page. I selected
"Options" and selected Heading 7 as a level 1 option (in addition to Heading
1). When I go to the preview page, I can see Heading 7. When I tell it okay,
and then refresh the TOC, the Heading 7 rows are not there.

If I go back to the TOC page, the Heading is no longer selected - so it
appears that it is not saving my selection.

How do it I get it to save?

The dialog box always resets; it doesn't pick up the current options from an
available TOC.

TOCs are maintained by TOC fields, and exactly what the TOC shows is controlled
by "switches". Press Alt+F9 to toggle on the field codes after you've created a
TOC that supposedly should pick up Heading 7 and copy what you see between the {
} field brackets to your reply to this message.

Also, please verify that you've actually formatted paragraphs with Heading 7 in
your document text, and that they aren't in textboxes or anything like that.

And lastly, please tell us which version of Word you're using :)

I am using Word 2003.

In this scenario I should have the following:

Heading 1 - level 1
Heading 7 - level 1
Heading 2 - level 2
Heading 3 - level 3

Here is what I see between the {}
{TOC \o "101" \h \z \u \t "Heading 2,2, Heading 3,3, Heading 7,1"}

According to what you show me here, the field shouldn't work at all. \o "101" is
totally unexpected. Are you sure you didn't mistype this?

This TOC should show Heading 7, but not Heading 1 (unless you've indeed made an
error at \o).

Try substituting the following in the TOC field and see if it makes a
difference:

{ TOC \h \z \t "Heading 1,1, Heading 2,2, Heading 3,3, Heading 7,1" }
