Sending attachments in Outlook 2003

When I send an email to members in-house (on the same
domain) with an attachment, such as a Word document, the
receiver may or may not get the attachment. It does not
happen all the time; it is random. If I send an email
with an attachment, outside the firm the reciever gets the
email with the attachment. Any idea how to correct the
problem?
