Probably the easiest is a table with some of its borders turned on and
others turned off.

As Jay says, there are a lot of ways to do this. If you're going to print
out the document, it really doesn't matter how you do it. If students are
going to be filling in the blanks online, a table is about the only good
way. For printed forms, I tend to use a paragraph border (you have to use
both Bottom and Horizontal Inside borders), then just press Enter to get as
many lines as desired.
