server extensions

I have published a page where I gather informaion from
users.

1.) How do I remove the "Frontpage Extensions..." title
from the top of the page?

2.) After submitting the information, it
reads "http://www..... does not support frontpage
sxtensions." I have another site through the same
company, so I know they do. How am I doing this wrong?

Please help.

If the form is sending data to an email address, then the host has not
configured the extensions to process email for this web site/account.

Note: FP extensions are applied and configured on a per account basis by
most host, so having the extensions work on once account, has no bearing on
any other account on that server.
