Set up form to be filled out and emailed

Frontpage 2003
I want to set up a form (announcement for a banquet) that would require the
reader to answer several questions. I then would like for them to email this
form to a certain address.

To create the form, you can just select insert, then form, then form again.
If you need any more help with the form design and field properties, let me
know. Now, right click anywhere in the form, and select form properties.
You should see an option here under "send to" that asks for an email address.
Insert the email address, and click ok, and you're done.
Note, I believe that your hosting provider needs to run support for
frontpage server extensions for this feature to run properly.

In frontpage, from the "file menu", select "new". Then in the right task
pane, under "new page", select "more page templates". Select one of
templates, and click ok.
