Signing in box keeps popping up

I have had this problem for some time and have no idea how
to get rid of it... any help gratefully recieved cos it's
driving me mad!!

When I click send/recieve the Outlook signing in box
"Please enter your user name and password for the
following server" pops up and if I press "Ok" it goes then
comes up again - if I press "Cancel" then it goes but comes
back again

Once it starts then it continues - if I right click the
message on my toolbar and "close" it goes for a bit but
then pops up again if I go on the internet or send/recieve
again

Any idea's?
