Slimline DVD Cover Label format, where to find please?

I am looking for a template that will allow me to print out DVD case labels
for DVDs that are in the "slimline" format, similiar to those found now on
the Windows XP magazine etc.

Use a ruler and create a table with cells of appropriate size - or much
better, use a proper labelling program, such as Surething which will give
you infinitely more control over the layout.
