Sort By Icon has a strange icon

I created a new form for my contact folder, and now I have some contacts
with an icon of a yellow folding page with a green push pin going trough in
the sort tool bar. The rest of the icons have the typical Vcard icon, the
card with a persons profile. Does anybody have any idea what this icon
means.

It means that your new form did not have the icon changed on its
(Properties) page.
