I am looking for a template for a Special Power of Attorney to Act "In Loco
Parentis". My children will be going with their Grandma for the summer and I
need something to send with them so she can take them to the Dr. if needed.

You're looking in the wrong place, I fear - this newsgroup is for technical
support of the database software Microsoft Access. Such a legal form would be
very, very different in various jurisdictions (and the internet spans the
globe). You'll do much better asking a local attorney, doctor's office or
legal aid society.
