sub-fields beyond a selection

I want to create a field in a form that gives the completor five choices, say
Word, Excel, Powerpoint etc.

When they check Word, that will then automatically open a new section where
they make more choices.

Example: If they enter Excel, that then opens a set of questions pertaining
to Excel. If they select Word, however, they will then be presented with the
section I creat regarding word.
