I need to have a form where a user selects from three options, for
example, Install, Change Request, Dismantle. Depending which option
they choose they are then presented with another form (a sub form?)
appropriate to the Install, Change Request or Dismantle.
So, just to be absolutely clear (at least I HOPE I'm being absolutely
clear), I need four forms. The first one is like a "cover sheet" from
which the user chooses one of thrtee options and then are taken to the
form for the option they chose.
