Survey Template

I want to know if Word has a template to create a document where I could have 2 columns of check boxes going down the left side of the page, my text in the center of the document, and a space to the right of the page to hand write a number. It should sort of resemble when you take a survey and you have to check by hand "yes" or "no" and then the question. Does anyone know what I'm talking about and could help me?
