Tab and Shift-tab doesn't change outline level, it just tabs over

At some point in my investigations over the last few days I did check that,
and found that the "start at" number was 2 and not 1. I changed it at the
time. I've just checked again and it is set to 1, even though the first
heading number is actually 2.
