Table of Contents - imcomplete/incorrect Updates

I am having trouble updating a Table of Contents in an existing template.
Now that I have added content, but when I go to update the TOC, I am getting
the following:
1) "Error bookmark not found" instead of page # - I have followed the
standard help text instructions for correcting this, but they have not worked
2) Page numbers out of alignment or incorrect spacing - I have been able to
correct some of this by re-formatting the headers in outline view. However,
the errors seem to crop up again and it is very difficult to identify the
source
3) Some of the headers are not updating when I've made changes in the body
of the document.

In response to these errors, I have reformatted the TOC multiple times,
trying headers, levels, and manual tags with no success. If you can make any
recommendations, I would be eternally grateful. The standard Help on these
issues is quasi-useless. Thanks so much.
