Table Of Contents problems, will not give me third outline

I have a large document with column breaks, page breaks and scanned images.
I can not get the third outline in my table of contents. I select the styles
I want in the TOC order and select 3 outlines and always get two!

It seems to always go back to the default style.

Any suggestions?
