Table to Form

I have information in a table that I want to link to the Form I created. I am
getting Name? error and the information isn't going in. What do I do?

What did you use for a Record Source?
Are the Control Source names identical to the field names?

I don't know how to designate a record source.?? Someone said something about
a relationship -haven't done that either.

On the control source option I hit the ... and selected the table and section
