Template for Avery ring binder plain tab indexes (8) 11507

I'd like a template to use for Avery Ring Binder Plain Tab Indexes (8),
11507. Tabs run perpendicular to the page on the right side, with binder
holes on the left.

sites, it appears these are meant to be written on by hand, not printed with
