template for selecting/criteria data center and bandwidth?

Is there a template for selecting a data center and bandwidth with defined
criteria? We are looking into getting space in a data center and buying
bandwidth [bandwidth would be with data center or third party]. I am speaking
of a Switch and Data type data center and bandwidth is 100Mbit-Gig so we are
not talking about home DSL criteria.
Template can be in Word, Excel, Visio...
