Is there a template for putting a picture and text onto a CD case, front and
back?

A CD case template is essentially a table with the cells the correct size.
Once you have that the procedure is essentially that at
http://www.gmayor.com/graphics_on_labels.htm. Note Publisher is much easier
to use for CD case labels, and Surething infinitely better than either of
them.
