template request

i have noticed in the template area of officeonline that there are some
templates that have been subimmted by request. i have attempted several
times to create a template for my 5 month old daughters immunization record,
but i can not seem to get it just right. does anyone know where i go to
request a personalized template? or can anyone help me create one, or create
one for me? i am sure that if someone could create a nice immunization
template, many people would download it, not just parents, doctors too.
please help me

I know that Works has (had?) a template for immunizations. If you don't have
Works, I'll bet you have a friend who does. Ask them to convert it to Word
or Excel for you. I'm not sure which format it was in - it's been a while
since I saw it and I have no need for it.

thanks so much. i checked my works for a template for immunizations, but i
could not locate it. any idea what it is listed as?

Hi Mom,

You may want to print out the Immunization Record from
http://immunize.org/catg.d/p2022pdf
or check with your state/local health organization for the form they use as the requirements can vary by locale, some locales do
have forms online for their area.

The International Certificate of Vaccination (Yellow book) is often available through the local automobile club offices, travel
offices or doctors. The 'yellow book' is 16 pages to record vaccinations/immunizations

I'll have to see if I can find that Works template on my other system this
evening.
