Templates and forms

I need help with creating a form as a template. Some one has presented me with a .dot document that lets you fill in little windows and when you complete these boxes, it shows the final document complete with the info from the little windows. I am not an expert in word, and would sure like so advice to what I need to learn to create these documents. Thank you in advance. Jennifer

you off to a good start.
