The quickest way to insert data in a LOG table

I am writing an application and need to insert 3 fields
into LOG table (A table that keeps a logs of all changes
to...)
My question: what is the simplest and quickest way to do
this using minimum code?

The spammers and script-kiddies have succeeded in making it impossible for
