To: field blank after message is sent

We are running Outlook 2003 and have the latest and greatest software updates
and every once and a while when a user sends a message the To: field comes up
blank. The intended recipient (s) still receives the message however the To:
is blank. This is causing confusion on whether or not the intended recipient
actually received the message. Any assistance on this would be greatly
appreciated.
