Toolbar customization won't stick

Running Outlook 2003 on Windows XP.

When I customize a toolbar, Outlook doesn't "remember" the change the next
time it opens.

In response to a similar question about lost customized toolbars after a
power failure in this forum last year, MVP Milly Staples said:

Close Outlook. Find and rename outcmd.dat to .old. Once you have done so,
reopen Outlook and it should open fine. You will need to rebuild any custom
toolbars or customizations as the file outcmd.dat controls how the toolbars
display. This file can be very fragile.

Similar advice was offered in the Install and Configure newsgroup.

Unfortunately, this trick isn't working for me. I know I'm working with the
right outcmd.dat file because it has the right time stamp on it. I renamed
it, reopened Outlook, customized, closed, and reopened -- none of the
customizations were there.

Any ideas WHY this is happening, and what other steps I might take?
