Toolbar Error - 02, SP 2

Once Outlook is open, if I click on the "Actions" button
in the toolbar I get an error message and it auto closes
the program. The detail shows a bunch of stuff, though it
means nothing to me. Tried auto fix w/ no luck. Tried
Restore to previous date, no luck. Confirmed all updates,
no luck.

Any ideas????
