Toolbar shortcut to Add-in

Hi all

Can someone please point me to a link or otherwise give me step-by-step
instructions on how to create a toolbar icon shortcut to an installed
add-in?

In addition to the specific query, is there a general way of creating a
toolbar shortcut icon for ANY menu item?
