Training Needs Assessment Questionnaire

I'm in the process of developing a questionnaire to determine training needs
within the organization. Does anyone have a general survey that they may not
mind sharing with me?

Strange as it may seem, the training needs of an organization depend
entirely upon the business which the organization conducts. A training
program ideally suited for the Navy Seals, for instance, might be
inappropriate (perhaps even a bit harsh) for a grammar-school ballet class.
