Here is something I got from their tech support:
"I do not know the specific problem in your case, but first you must
understand that MS Publisher will not export your file at the proper
resolution of 300dpi, which means your cover is probably coming out
undersized. In addition, your cover file PDF, since it is being
exported from MS Publisher, will have layers. The PDF cover section
cannot handle layered files, and when it encounters one, it gives an
erroneous "wrong size" message."
"Most likely you are running up against both of these problems. Covers
are meant to be images, and they are best created in image editing
software. MS Publisher is meant to create documents, and converting a
document to an image is always a troublesome proposition."
