Type of box for data entry form

I'm a newbie and I've run into an obstacle...

I'm creating a data entry form from a survey. One of the questions is a
list of 12 reasons why the person cannot attend meetings. The person rates
each reason from 1 to 12, with 1 being the most often reason and 12 the least
often reason. What is the best way to enter and compile this type of
information?
