Updated Form Template Misbehaving

Hi,
I updated a form template that accesses the Sharepoint User Profile Service
to pre-populate user identification fields throughout the form. This action
worked prefectly for many months, but after the update, the one user of this
form is now getting MY user info in his form rather than his?
How is this possible? What can I do to troubleshoot this?

Reminder: When accessing the UserProfileService to get the logged in user
data, don't put your domain\username in the data connection (leave it
blank).
