User an Unbound Form to generate a report and chart set up as a subreport

I created a report that based on the following parameters in a form will generate a report: Customer Number, Customer Name and Start and End Dates. I want to add a chart to the report as a subreport and want it to display based on the same criteria entered in the form. Can you tell me how to do this?
