User defined actions doesn't appear

Hi,
I'm creating user defined actions in a form and responding to it
using vba's event CustomAction. Everything works fine, but...
All actions are defined to be show both in the menu and in the
control bar but sometimes they just don't appear, or appear just in the
menu. I really don't know what's going on.. any ideas ?
