User permissions vanishing from a PWA site

Recently, we have had a few user permissions vanish from a few different PWA
sites. For one user, I added him back to the site last week, but got another
email from him a week later saying he lost access again.
Is there some reason why permissions would vanish like that? I don't think
anyone is changing permissions on the site. Is there a way to see if someone
is making changes?

Permissions for Project Workspaces ultimately come from the Project Server
permissions. When you add permissions directly to a project workspace,
those permissions will be removed when the individual's PWA permissions are
changed.
