Vendors for building order form

I'm looking for a vendor that can create a password protected subsite that
our distributors and contractors can log into, place an order without
necessarily paying for it but acting like a shopping cart, and having that
order e-mailed or in a printable format so it can be faxed. We will have one
listed price but the log in will determine the discount they will receive
when completing the order. We have 5 levels of discounts or multipliers as we
label them.

Does anyone know a good vendor that can create this for my company?
