If you are looking for the 97 template, it is part of Access 97. Select New
and then Databases. It's toward the bottom of the list

If you are looking for Access 200x databases, try the template gallery at
http://office.microsoft.com/en-us/templates/default.aspx.

The first two hits when you search for video database are a video collection
database and a DVD collection database.

Do you have Access 97?
It's in with your database templates
File + New + General Templates

If you have Access 2000 or newer, try the Microsoft Template gallery:
http://office.microsoft.com/en-us/templates/default.aspx
Enter Access in the Search box. It's the Music Collection Database
