Visio Report on Activity's Functional Band

Hi,
We are trying to create a Visio Report that we call an "Activity and
Responsibility Matrix". We want to be able to display in an Excel-Report
which department/functional band that performs which activity. This far, I
have only managed to get the activities sorted by numbering, but can not
cross refer to which functional band they are located within. Example:

Activity Functional Band
"Send report" "Business Units"
"Receive report" "Head Quarters"

Help would be much appreciated!
