Web Forms: two work, one doesn't

I've had my website up for about 8 months now. This forum helped me a lot
when I was publishing it for the first time. I hope you can help now.

My dilemma:

The following form isn't working (always has in the past - now I get a
message that says "HTTP 404 Not Found"):

While the following two forms are working:

http://www.somoscapazes.org/index_files/donar.html

The one that isn't working happens to be the one that usually gets the most
use and is at this moment the most important to me.

I designed the site with Publisher 2003. I have always used FTP to publish.
Normally, to make the forms work, once I update and re-publish my website, I
simply uninstall, then re-install Microsoft Front Page Extensions, and no
problem. I have attempted this a number of times with the problem form and it
just isn't helping.

I have attempted to access the following pages, which have been so highly
recommended by David Bartosik to previous posters on similar subjects, but a
page comes up telling me "You are not authorized to view this page":

I have also looked over the following suggested pages, but I don't think
they apply to me as I've always been able to make the forms work by using
FTP, and 2 of the 3 forms do work:

Please help! There are a number of people trying to use this form at the
moment and it's important for my non-profit organization to present a fully
navigable website.

Since I posted this message a few minutes ago, I have received a couple of
forms (the one that was/is having the problems) filled out by people on my
website, in my e-mail inbox. However, it still isn't working from my computer
and someone just called me saying that it wasn't working for her.
So it works from some computers and not from others. Does this make sense?
What's wrong?
Could it be that the forms I received in my inbox were sent hours ago when
there perhaps was no problem and I've just received them now?

Emilie,

I have no clue if this is the root of "your" problem but it is not
unusual with some forms. There is a adware/spyware/virus program called
either FunWebProducts or WebFunProducts that can seriously screw with
the ability to submit forms. It's not really malevolent otherwise, and
commonly is installed when one decides to install those 20 gazillion
smilies that are attached to a lot of messages.

On the forms where I use Soupermailer as the script, it will not work if
this program is installed.
