Hi,

I am adding an order form to my website and I am running into a problem
where it is requiring visitors to have a user name and password to submit
their form. Do you have any suggestions abotu how to remove this?

Have you contacted your web host? Also, try creating a new page and a
simple form on it and see if the same problem happens
