What application was this pdf document originally made in?

Publisher would be the most common Microsoft program that will do this but
it could have been created in Word or PowerPoint as MS created products.

Any desktop publishing program like Publisher will do this the easiest as
they are page layout programs. A free one that is available is Serif's Page
Plus SE free edition available at
http://www.freeserifsoftware.com/software/pageplus/index.asp.

As for creating the PDF file, if the program that is used can't save
directly as a PDF file, any "print driver" PDF creator program can. Another
freebee that I use is Primo PDF creator from

While it can be tedious to work on text-heavy pages like this in PowerPoint,
you could create it there if you like.

If you have Publisher, that'd be a good choice as well (and probably easier to
work with in this case).
