Where can I find an already written Patiuent history Access databases?

Hi,

I am looking for an access d/base for up to 1000 patients in a small
chiropractors office.

Fields with the normal stuff one would input instead of writren cards
and have the ability to search, sort and print.

Prefer standalone if possible altho' doesn't realy matter I guess as I
do have XP Access

Any suggestions?
