Where can I get a template for DVD case inserts (not CD)

I can find dozens of word templates for creating CD case inserts for a CD
case (jewel case), but I need a template for the DVD case (larger than a CD
case).

Measure a DVD insert then create a table with three cells that match the
size of the three parts of the DVD case cover on a landscape oriented page.

Much better - treat yourself to a copy of Surething CD labeller which makes
this task a breeze.
