Where is the opportunity form in BCM 2007

I just installed Office 2007 professional with BCM trial edition. All works
well, except that when I attempt to create a new business opportunity, all I
get is the basic task form. What happened to the detailed opportunity form
that was available in 2003?

The Outlook form cache is probably corrupt. Try clearing it.
