which program should i use for a questionare with drop down aswers

I want to make a simple log with a few multiple choice answers. What I
really want it for is to make an inspection sheet and to be able to use it on
my tablet pc and walk around and tap the drop downs to fill in my answers.
for example, type of roof .... drop down would have choices of tile, rock
...etc
